A POLST (Portable Order for Life-Sustaining Treatment) is a medical order signed by a physician. It turns a patient's wishes for life-sustaining treatment into specific, actionable instructions. It covers decisions about CPR, ventilators, artificial nutrition, and hospital transfers.

How a POLST Differs from a Living Will

A living will is a legal document that states general end-of-life preferences. It typically requires two physicians to confirm a terminal condition before it takes effect. A POLST is a medical order that applies right away. Paramedics, emergency room staff, and nursing facility providers follow it without delay.

Who Should Have a POLST

A POLST is generally appropriate for people with serious advanced illnesses. It also fits those with chronic conditions likely to need emergency care. People with frailty related to advanced age may benefit too. Healthy adults typically do not need a POLST. A living will and healthcare power of attorney are usually enough until health changes.

In Arizona, POLST forms can be registered with the statewide health care directives registry. This is allowed under A.R.S. 36-3291. Registration makes them available to providers electronically during emergencies.
